Abstract: 

Objective: There are several studies reporting the therapeutic effects of Berberis vulgaris on liver diseases. This study was done with the purpose of examining the effect of B. vulgaris oxymel (BO) in patients with refractory primary sclerosing cholangitis (PSC) and primary biliary cholangitis (PBC), who did not respond to current treatment. Materials and Methods: Patients with PSC or PBC who were receiving ursodeoxycholic acid (UDCA, 13-15 mg/kg/day) for at least six months, but their serum levels of alkaline phosphatase (ALP) were still 1. 5 folds higher than the normal upper limit during the last six months, were asked to participate in this quasi-experimental study. Patients were asked to take 0. 5 ml/kg/day of BO two times a day for three months along with UDCA. At the end of the study, serum levels of ALP, aspartate aminotransferase (AST), alanine aminotransferase (ALT), gamma-glutamyl transferase (GGT), total bilirubin (TB), direct bilirubin (DB), and creatinine as well as prothrombin time (PT), international normalized ratio (INR) and quality of life (QOL) based on PBC-40 questionnaire were assessed as outcomes. Results: Our results showed that BO notably attenuated the serum levels of ALP, AST, ALT, GGT, TB, and DB, as well as PT and INR and significantly improved QOL. Conclusion: For first time, we showed that additional therapy with BO has a promising effect in the treatment of refractory PSC and PBC.

Abstract: 

The medicinal plants from genus Berberis are particularly important in traditional medicine and the food basket of Iranians. Given various plants from genus Berberis and their economic, nutritional, and medicinal status in Iran, this study seeks to investigate the findings of recent studies on the phytochemical characteristics, specifications, and uses of Berberis vulgaris. In this review article, 350 articles were initially retrieved from reliable scientific databases using relevant search terms. Then, 230 articles were selected and 120 were excluded after a primary analysis. Finally, 98 articles related to the subject under study were meticulously examined and the required data were extracted and classified according to the research purposes. The findings were divided into eight separate sections: Introducing Berberidaceae family, different species of Berberis, pharmaceutical organs, B. vulgaris nutrition facts and minerals, the antioxidants and alkaloids compounds in fruit and other organs, action mechanisms of preventing and treating diseases, traditional uses of B. vulgaris, and its properties reported by recent studies. The results briefly indicate that B. vulgaris contains a large number of phytochemical materials including ascorbic acid, vitamin K, several triterpenoids, more than 10 phenolic compounds and more than 30 alkaloids. Therefore B. vulgaris may have anti-cancer, anti-inflammatory, antioxidant, antidiabetic, antibacterial, analgesic and anti-nociceptive and hepato-protective effects. Regarding the use of different organs of B. vulgaris in traditional medicine and their confirmed effects in the recent studies, it is possible to use different organs of B. vulgaris, especially fruit, to develop new drugs.

Abstract: 

Cardiovascular disorders are the leading cause of mortality worldwide. Berberis vulgaris (B. vulgaris) is a commonly used plant in traditional medicine. In recent studies, B. vulgaris showed antiarrhythmic, antihypertensive, anticholinergic, and cardioprotective effects. We reviewed the literature to explore the possible prophylactic and therapeutic roles of B. vulgaris in cardiovascular medicine. A computer literature search was conducted to identify all relevant studies that have investigated the role of B. vulgaris in prevention or treatment of cardiovascular diseases. We also searched the citations of the retrieved articles. Using a systematic approach, we conducted a scoping review that included a total of 37 articles. Twelve studies examined the antihypertensive effects of B. vulgaris, seven studies investigated its antiarrhythmic effects, while its inotropic and cardioprotective effects were evaluated in four and eight studies, respectively. B. vulgaris showed a beneficial effect in reducing blood pressure, enhancing cardiac contractility, and protection from reperfusion injury. However, the mechanisms of these effects are still under investigation. Moreover, it could modify major risk factors for cardiovascular disorders, such as oxidative stress, hyperglycemia, and hyperlipidemia. Further studies are needed to translate these findings into effective cardiovascular medications.

امروزه به دلیل اثرات نامطلوب و عوارض جانبی داروهای صناعی و همچنین به دلیل طبیعی بودن، خطرات و عوارض کم، در دسترس و ارزان بودن گیاهان دارویی نسبت به داروهای سنتتیک داروهای گیاهی، مصرف زیادی توسط مردم دارند. کاربرد روز افزون گیاهان دارویی، لزوم انجام تحقیقات بیشتری را بر روی گیاهان طبی ایجاب مینماید. تاثیر مثبت و استفاده صحیح از گیاهان دارویی مشروط به داشتن اطلاعات دقیق و علمی در مورد نحوه و زمان و مقدار مصرف آنها است. گیاه زرشک (Berberis vulgaris) به طور وسیعی در کوهستان های شمال شرق ایران رویش دارد و به صورتها مختلفی مورد مصرف قرار می گیرد. اثرات دارویی زیادی برای این گیاه ذکر شده است. زرشک با مهار آنزیم ACE (آنزیم مبدل آنژیوتانسین) در مغز یا خاصیت آنتی اکسیدانی خود میتواند باعث کاهش علایم بیماری پارکینسون شود. در بررسی های فتوشیمیایی انجام شده نشان داده شد که زرشک دارای فعالیت آنتی اکسیدانی است و میوه زرشک دارای ویتامینC  و مالیک اسید است، عصاره زرشک، مونوآمین اکسیداز MAO-A را مهار می کند، بنابراین سطح مونوآمین ها مانند اپی نفرین و دوپامین در مغز موش افزایش می یابد که اثر ضدافسردگی دارند. نمونه هایی از آلکالوئیدهای این گیاه عبارتند از بربرین، اکسی اکانتین، بربامین، برولسین و... مروری بر مطالعات، نشان داد که میوه گیاه بربریس ولگاریس دارای خاصیت ضد فشارخون است که از طریق سه ماده بربامین، بربرین و اکسی اکانتین اثرات ضد فشار خونی خود را اعمال مینماید. زرشک سیاه در کبد رات غلظت LDL-C و نسبت کلسترول تام به HDL-C را کاهش میدهد با توجه به نتایح مشخص شده عصاره زرشک دارای اثر بخشی هم در کاهش قطر زخم لیشمانیا (P<0.001). و هم کاهش تعداد انگل در ضایعه میباشد (P<0.001). عصاره گیاه زرشک قادر است التهاب ناشی از تزریق فرمالین در کف پای حیوانات را به طور معنی داری کاهش دهد. از دیگر اثرات دارویی زرشک میتوان به آنتی استیل کولینستراز، آنتی ارتریت، آنتی باکتریال، آنتی کارسینوژنیک، آنتی نئوپلاستیک، آنتی پریستالتیک، آنتی سپتیک، آنتی اسپاسمودیک، اثر ضدتهوع, ضد هلیکو باکتر، ضدالتهاب، ضدخارش، ضدتب، ضدقارچ، محرک ترشح صفرا، عرق آور، ادرار آور، هپاتوتونیک، ملین (ضدیبوست)، مسکن، نیروبخش، اشاره کرد. با توجه به نتایج حاصل از تحقیقات میتوان از ماده موثر زرشک برای درمان و کنترل بیماریهای پارکینسون، افسردگی، فشار خون، چربی خون بالا، سالک، یبوست، هموروئید سنگ کلیه، بیماریهای آمیبی، الزایمر، بی اشتهایی، دردکمر، سنگ صفرا، ترشح زیاد صفرا، خونریزی، برونشیت، سرماخورگی، سرفه، کرامپ، دیابت، اسهال، اختلالات هضم، تب، فیستولا، ژیاردیوزیس، سردرد، حمله قلبی، هموروئید، هرپس، التهاب، بیخوابی، خارش، مالاریا، تریکومونیازیس و استفراق استفاده کرد.

Abstract: 

Background: Surgery is one of the best choices for the treatment of hydatidosis. The use of effective scolicidal agents during surgery for hydatid cyst is essential to prevent the secondary infection. Up to now no effective and safe agent has been identified for this purpose. Berberis vulgaris called “Zereshk” in Persian has been traditionally used as herbal remedy for the treatment of complaints and it is widely cultivated in Iran. Many studies have shown that it has antibacterial, antifungal and antiparasitic effect.Methods: In this study the scolicidal effect of Berberis vulgaris aqueous and hydroalcohol extract for different concentrations (for aqueous: 0.5, 1, 2 and 4 mg/ml and for hydro-alcohol: 0.25, 0.5, 1 and 2 mg/ml) at different exposure times (5, 15 and 30 minutes) was evaluated. For this purpose, we obtained liver hydatid-cysts from a slaughter house. Viability of protoscolices was assessed by 0.1% eosin staining. Normal saline and hypertonic saline were used as negative and positive controls respectively.Results: All the different concentrations of Berberis vulgaris aqueous and hydroalcoholic extracts had scolicidal effect. An aqueous extract with 4mg/ml concentration acted as positive control and we observed to lead to the death of 100% of protoscolices in the first 5 minutes. The least scolicidal effect (12.3%) was observed in an aqueous extract with 0.5 mg/ml concentration. The scolicidal activity of hydro-alcoholic extract with concentration of 2 mg/ml was 100% after 5 min of application, which was the same as positive control group. We noticed a significant increase in protoscolicidal activity with an increase in concentration in the two extracts of Berberis vulgaris (P<0.001).Conclusion: It is important to mention that all the concentration levels and exposure times applied in this experiment are relatively low, since scolitical activity in both of the extracts is at its highest in this low spectrum. For further experiments, we suggest that the stability of cyst fluid in both of the extracts should be assessed. Therefore, after In vivo examination and additional experiments, it may be used as a suitable and effective scolicidal in surgery.
